Dipyridamole and Headache – Does Dose Titration Help?

Background

Headache was reported by about one third of patients using dipyridamole after stroke or TIA in ESPS 2. The headache is often more intense during the initial phase of treatment (see figure below). Different treatment regimens have been suggested to try to decrease the prevalence of headache (e.g. prophylactic or symptomatic treatment with analgesics, or a titration period with an initially lower daily dose of dipyridamole). However, there are no published studies reporting the effects of such treatments in patients with ischaemic cerebrovascular disease. A pilot study to examine if an initial titration of dipyridamole results in a lower prevalence of headache was performed.
