Germany: DSG/DGN Guidelines 2005 (excerpts):
ASA or ER-DP + ASA*:
Treatment with the combination of extended-release dipyridamole plus ASA (200/25mg twice daily) is first line treatment for patients with a high risk of recurrence (≧ 4%/year) (**).
ASA:
Recommended daily dose of 50–325 mg ASA. Lower doses of ASA (50–100 mg) have fewer side effects and are equally effective.
Clopidogrel:
Clopidogrel (75 mg) is given in patients who cannot tolerate ASA (*) and in patients with cerebrovascular disease and concomitant other vascular disease (**).
Clopidogrel + ASA:
Patients with TIA or stroke and unstable angina or non Q-wave MI should be treated with a combination of clopidogrel plus ASA (75/75 mg) (**).
Ticlopidin: Reserve medication due to side effects.
TIA or stroke during ASA therapy:
There are no studies. Recommendations: Dipyridamole plus ASA twice daily, clopidogrel (75 mg once daily), anticoagulation with INR of 2.0-3.0.
Duration of therapy:
There are no studies exeeding 4 years of treatmet. Therapy should be life-long, theoretically, if tolerated.
