Earlier studies of dipyridamole in stroke prevention were small, statistically underpowered, and inconclusive. The maximum dose used in most of these studies was 75 mg QID (300 mg/d) of immediate-release dipyridamole. Findings regarding dipyridamole's pharmacologic effects together with the known benefit of adding dipyridamole to warfarin so as to prevent thromboembolism in patients with mechanical heart valve implants led to a larger stroke prevention trial using higher daily dosages.
